Community Integration Program

The Community Integration Program works to increase the scope of available life-skill services offered to survivors through the Club. The main facet of the Community Integration Program is referrals and assistance in contacting other local organizations to maximize support. Transportation, as well as all other aspects of life and health can be addressed and the appropriate connections made.

Through well developed partnerships with the Erie St. Clair Community Care Access Centre, the Chatham-Kent Health Alliance, the Friends of Stroke, Southwestern Ontario Stroke Strategy and Chatham-Kent District Stroke Centre the Club is striving to assist all survivors move seamlessly back into the community.
